ONLINE BILL PAY

Conveniently and securely pay your bill using your VISA, Mastercard, Discover or American Express with no additional fees.  A receipt will be emailed to the address you enter when making payment. If you have questions about your bill or are unable to pay the balance in full at this time, please call the Claris Eye Care office at (952)522-4900.

We provide a variety of payment options for patients with private insurance, employer-based insurance, public coverage and those who self-pay. Whatever your circumstance, we are here to provide a confidential review of your options.  

INSURANCE

We contract with most major insurance carriers in our region.  Below is a general list of insurance carriers we accept.  This list is not all-inclusive and is subject to change.  We do not accept vision plans.  Always bring along your insurance card for each appointment.

Even though we work with these carriers, your policy may have coverage restrictions such as network, referral or prior authorization requirements.  To find out the specific details of your insurance plan, please call the customer service number on the back of your insurance card.

SELF PAY OPTIONS

We accept payment by cash, check, credit card, health savings account (HSA) or flex spending account (FSA).  Financing with approved credit is available for procedures not covered by insurance.  Copays are due at the time of service and statement balances are due upon receipt.  A full copy of our financial policy can be found here.

FINANCING OPTIONS

We offer financing with approved credit through Wells Fargo. They provide extended payment plans for procedures not covered by health insurance.
